1. Call any of our 17 Gwinnett Clinic locations. Same or next-day appointments are usually available with our primary care physicians by phone only. Appointments to see our specialists may be available within 1-2 days by phone as well, if there is availability!
2. Complete the below form. Online requests can be made for appointments at least 72 hours in advance. Please note, this form is only a request. You will receive an email or phone call to confirm the appointment. If you do not hear from us within 48 hours of submitting your request, please call your nearest Gwinnett Clinic office.
